NTUC Health Co-operative Ltd. is seeking a dedicated, compassionate, and outgoing Case Associate to join its community mental health (CREST) team for islandwide outreach and client support. Offering a competitive monthly salary ranging from $1,600 to $2,600 on a permanent full-time basis, this entry-level executive role involves supporting community seniors, conducting mental health and dementia awareness outreach, providing care coordination, and monitoring the well-being of vulnerable clients in the community.
As part of the CREST team, you will play a vital role in supporting clients with or at risk of mental health conditions and/or dementia, linking them to necessary community resources, organizing preventive programmes, and collaborating with regional health services, hospitals, volunteers, and grassroots organizations.
Key Responsibilities
- Care Coordination & Service Linkage: Provide care coordination and connect clients with or at risk of mental health issues and/or dementia to appropriate community resources, support networks, and caregiver support services.
- Community Outreach & Awareness: Plan and conduct community outreach to faith-based, corporate, and grassroots organizations to promote mental wellness and reduce stigma.
- Case Finding & Screening: Plan and execute targeted mental health and dementia screenings to promote early identification and facilitate timely intervention.
- Preventive Programs & Visits: Organize structured preventive activities for seniors and conduct regular well-being visits to monitor assigned clients in the community.
- Assessment & Intervention: Support initial assessments of walk-in cases and referrals, following up closely with prescribed care and intervention plans.
- Evaluation & Documentation: Maintain accurate case documentation, evaluate programme outcomes, and support occasional weekend outreach events.
