Job Overview
Tan Tock Seng Hospital Pte. Ltd. (TTSH) is seeking a compassionate, dedicated, and detail-oriented Social Work Associate to support the Medical Social Services team at Jalan Tan Tock Seng. In this role, you will assist Medical Social Workers in gathering patient social histories, conducting psychosocial and care assessments, facilitating group work, and engaging community partners to ensure comprehensive patient welfare and discharge care.
Key Responsibilities
- Conduct patient and family interviews to collect biodata, socio-economic backgrounds, and psycho-social profiles.
- Collate and present essential social data to Medical Social Workers to facilitate effective interventions and multidisciplinary care planning.
- Manage routine social work cases independently in line with department standard operating procedures.
- Perform home visits and field assessments to evaluate patients' home environments and long-term care needs.
- Assist in organizing and co-facilitating psycho-educational and support group work sessions for targeted patient cohorts.
- Support community engagement initiatives, including community needs assessments, community mapping, and co-planning community outreach events.
- Assist in planning, coordinating, and implementing patient welfare projects and social work programmes.
- Uphold safe care standards, participate in clinical quality improvement projects, adhere to professional Codes of Ethics, and observe infection control protocols.
Requirements & Qualifications
- Recognized Diploma in Social Work, Psychology, Community Services, or related social science disciplines.
- At least 1 year of relevant working experience in a healthcare, hospital, or social service agency setting.
- Strong interpersonal, interview, and active-listening skills with an empathetic mindset.
- Good verbal and written communication skills for comprehensive case documentation.
- Responsible, resourceful, and capable of working effectively in a multidisciplinary hospital environment.
